The PREACT<\/strong> project is a targeted and innovative European initiative that directly responds to the priorities of the EU4Health Programme, aiming to reduce the burden of Cardiovascular Diseases (CVDs) and Type 2 Diabetes (T2D) through prevention, health promotion, and the adoption of healthier lifestyles. PREACT brings together academic and clinical partners from Greece, Bulgaria, Cyprus, and Ukraine, while actively engaging vulnerable population groups such as Roma communities, Pomak women, people with mobility impairments, refugee families, and Ukrainian citizens affected by war. Based on a solid and data-driven needs analysis, the project addresses alarming trends in public health, including the growing prevalence of diabetes, obesity, insufficient physical activity, and tobacco and alcohol use\u2014factors that contribute significantly to the development of CVDs and T2D across Europe.<\/p>\n To meet these challenges, PREACT introduces a comprehensive and user-friendly web-based platform designed to serve adolescents, young adults, and the general population. The platform provides free and easily understandable health information, self-monitoring tools, and practical guidance for lifestyle improvement, all accessible regardless of age, ethnicity, educational level, or physical ability. It includes evidence-based content such as animated videos, questionnaires, and simple clinical assessments, empowering users to recognize risk factors and take proactive steps in managing their health. Additionally, the platform will support healthcare professionals with updated guidelines and good practices through the PREACT e-Handbook, reinforcing knowledge transfer and fostering sustainable health education across sectors.<\/p>\n A pilot phase will be implemented to ensure that the platform is inclusive, accessible, and truly useful for both the general public and disadvantaged groups. Feedback from pilot participants will shape the final version of the platform. PREACT also includes a robust dissemination strategy aiming to engage thousands of students, educators, healthcare professionals, and members of the public across Europe. With support from local municipalities, educational authorities, and academic institutions, the project is positioned to achieve wide-reaching impact. Ultimately, PREACT strives to bridge health gaps, improve early detection of chronic diseases, and promote a culture of prevention and wellbeing throughout the European Union and beyond.<\/p>\n<\/div>
